This web page contains everything you need if you would like to request use of Southminster facilities for your meeting or event.  Print a copy of each form, fill them out then send or bring to the Church Office on the second floor of the Christian Education Building.

Procedures for Using Southminster’s Facilities form:  You are one of many groups requesting use of Southminster's facilities for your event or meeting.  We are asking you to review this form then sign and return the second page keeping a copy for your files.  The second page must be completed and returned with the Request For Room or Equipment Form at the beginning of each Church year in September in order for your event or meeting to be scheduled.

Request for Room or Equipment Form:  You can request use of Southminster's facilities using this form, it contains the information Southminster needs to properly respond to your request.  The signed second page from the Procedures for Using Southminster Facilities  along with this form must be submitted together before approval is given and your event or meeting is scheduled on the Church calendar.
